Insurance
Modern Outlook is an affiliated provider with Restore Therapy LLC, and is considered a contracted rendering provider under the following insurance plans. You will be informed prior to services whether your sessions will be able to be billed to your insurance plan based on medical necessity.

We are happy to provide you with an itemized superbill so you can easily self-submit claims to your insurance for out of network reimbursement. If you would like us to submit claims to your insurance on your behalf, there is an added $40 charge per service. Additional travel fees may apply for on-site sessions.If you’re using insurance to help pay for therapy, check your benefits carefully—you are ultimately responsible for the full cost of therapy. There is no out-of-pocket expense for OHP members, including the billing charge.
We are out-of-network for all other insurances, and we do not take the Legacy Health Plan at this time.
We do not accept Medicare. If you have Medicare and Medicaid (OHP/ HealthShare), you may find a provider who takes Medicare. OHP/Health Share must be your primary insurance to work with Restore. If it is your secondary insurance, OHP will not cover services. They require you to use your primary insurance for all services, as OHP is a payor of last resort.
We accept cash, check and credit card and bank transfer. We require all new patients to have a credit card on file to hold appointments. If we do not yet have your card on file you may enter it here.
